Refer an Adult Age 55+ to CEED
CEED Transitional Living Community welcomes referrals for men and women age 55 and older who need safe, supportive transitional housing and are capable of independent living.
We serve older adults experiencing homelessness or housing instability, including veterans and returning citizens.
Our goal is to provide a stable living environment while helping residents maintain their independence, connect with community resources, and work toward long-term housing stability.

CEED welcomes referrals from community professionals and organizations, including:
• Hospital discharge planners
• Social workers and case managers
• Skilled nursing and rehabilitation facilities
• Veteran service organizations
• VA professionals
• Senior-service organizations
• Community mental-health providers

Basic Eligibility
• Be 55 years of age or older
• Need transitional housing
• Be capable of independent living
• Be independent with their activities of daily living (ADLs)
• Be able to manage their own medications
• Be appropriate for a shared-living environment

• Complete CEED’s screening and intake process
All referrals are reviewed individually. A referral or application does not guarantee admission. Admission is based on eligibility, program fit, successful screening, and current housing availability.
Understanding CEED’s Level of Support
CEED is a supportive transitional housing program for independent adults age 55 and older.
CEED does not provide skilled nursing care, medication administration, personal-care services, or 24-hour medical supervision.
Residents may continue receiving services from their own healthcare providers, behavioral-health providers, case managers, veteran-service providers, and other community professionals while living at CEED.

Step 2 — Submit Referral Information

CEED will provide information regarding the documents needed to begin the referral and screening process.
When applicable, referral information may include:
• Basic demographic and contact information
• Identification
• Income or benefits information
• Current housing situation
• Relevant medical information
• Current medication information
• Mobility and ADL information
• Case manager or referral-source information
• Discharge information, when applicable
• Other documentation needed to determine program eligibility
Step 3 — CEED Reviews the Referral

Our team reviews the information provided to determine whether the applicant appears to meet CEED’s basic eligibility requirements and whether CEED can appropriately meet the individual’s housing needs.
Step 4 — Screening & Intake

Eligible applicants complete CEED’s screening and intake process.
This gives CEED and the prospective resident an opportunity to discuss the shared-living environment, resident expectations, individual needs, available supports, and program requirements.
Step 5 — Admission & Move-In

If the applicant is accepted and housing is available, CEED will coordinate the admission and move-in process with the resident and referral professional, when applicable.

CEED values strong partnerships with the professionals and organizations supporting our residents.
With appropriate authorization, we welcome continued collaboration with case managers, social workers, healthcare professionals, behavioral-health providers, veteran-service professionals, reentry professionals, and other members of a resident’s support network.
Our goal is to help create a coordinated transition into CEED and support each resident as they work toward greater stability and independence.
